Summa Western Reserve receives Get With the Guidelines Gold Plus Quality Achievement Award.

Summa Western Reserve Hospital has been awarded the American Heart Association Get With The Guidelines Gold Plus Quality Achievement Award for its heart failure treatment services. The recognition signifies that Summa Western Reserve Hospital has reached impressive benchmarks in the evidence-based treatment of heart failure patients.

“The American Heart Association Gold Plus Award for the heart failure program at Summa Western Reserve Hospital is a culmination of years of hard work by the nurses, social workers, dieticians, therapists, and physicians,” says Dr. Stephen Heupler, a cardiologist for the Summa Western Reserve Hospital Heart Failure Program. “We recognize that heart failure management requires a multi-disciplinary approach to obtain the best results.  Healthcare workers here at Summa Western Reserve Hospital appreciate this and have worked together to provide our patients the best and most optimal care for their congestive heart failure diagnosis.  As a result, our patients have had excellent outcomes relative to other institutions. We are proud of our achievements, but will continue to improve our care and anticipate even better results in the future.”

Summa Western Reserve Hospital earned the award for demonstrating an 85 percent or higher adherence to all the Get With The Guidelines–Heart Failure Quality Achievement indicators for two or more consecutive years and for achieving 75 percent or higher compliance with four of nine heart-failure quality measures to improve patient care and outcomes. The hospital received the Silver Plus award in 2011 as it continued toward consistent successes in heart failure care.

Throughout the initiative, Summa Western Reserve Hospital has developed and implemented acute and secondary prevention guideline processes.  The program provides hospitals with a web-based patient management tool, decision support, robust registry, real-time benchmarking capabilities and other performance improvement methodologies toward the goal of enhancing patient outcomes and saving lives.

This high-tech, evidence-based approach enables Summa Western Reserve Hospital to improve the quality of care it provides heart failure patients, save lives and ultimately, reduce healthcare costs by avoiding re-hospitalization.

According to the American Heart Association, about 5.7 million people suffer from heart failure.  Statistics also show that, each year, 670,000 new cases are diagnosed and more than 277,000 people will die of heart failure. Get With The Guidelines is a quality improvement initiative that provides healthcare providers with tools that follow proven evidence-based guidelines and procedures in caring for heart failure patients to improve outcomes, prevent future hospitalizations, and prolong lives. Using these guidelines, heart failure patients undergo aggressive risk-reduction therapies, such as cholesterol-lowering drugs, beta-blockers, ACE inhibitors, aspirin, diuretics and anticoagulants in the hospital. They also receive alcohol/drug use and thyroid management counseling as well as referrals for cardiac rehabilitation before being discharged.
